The first and only pediatric SNF in Washington State!

Bridges to Home is a facility for children, teens, and young adults who are coping with the challenges of a catastrophic injury, chronic illness, medical complexities, or congenital syndrome.  Bridges to Home is an innovative, transitional care alternative aiming to reduce the length of hospital stays.  We are dedicated to supporting families and caregivers by providing education, building support systems, coordinating services, nurturing development, and reuniting kids with their home communities. 

Services

• Physician Rounds

  • Weekly interdisciplinary physician rounds at Bridges to Home to monitor and adjust medications and interventions.

• Complex Care Coordination

  • Coordinating equipment evaluations and ordering from DME suppliers. 

  • Respiratory therapy oversight provided in the facility.

  • Scheduling and coordinating specialty clinic physician visits.

• Caregiver Education  

  • We provide flexible training and take as much time as needed to ensure caregivers feel confident in providing high quality care to their child.

• After Care Support

  • Coordination of care services and nursing supervision, included in aftercare support up to 6 months post discharge.
